This presentation focuses on treatment escalation plan conversations and is intended to guide clinical decisions and not result in a definitive triage decision – unless the patient is clear about limits of treatment OR the clinical situation is such that hospital admission (other than for unmanageable symptoms in the community) would not be appropriate.
The aim of treatment escalation plans are to guide those involved in their care related to their wishes, what is important to them and identify the level of intervention or ceiling of care. This is not a legal document but provides valuable information in the decision making process. It needs to be visible and accessible to those caring for them.
This 19 minute presentation will be very helpful as you prepare to do this work and covers:
- the importance of TEP conversations
- recognising the challenge of providing this support when unable to offer it in person.
- walks step by step through typical scenarios
- demonstrates useful phrases
- shows how the TEP form should be completed.
- considers the holistic needs of patient, family and professionals
